Who is the plan for?
Our plan is designed for everybody who wishes to attend the practice on a regular basis and have peace of mind that their preventative dental care is covered.
What if I need to see the hygienist more often?
We have different plan options available tailored to your needs. Please do not hesitate to speak to your dentist to find out which option would be most appropriate for you.
How to Join?
Joining is quick and easy and there is no need for an assessment. All you have to do is complete a Direct Debit mandate. This can be done either online here, over the phone or visiting us at Elstead Dental Surgery.
An initial registration fee of £8 will be charged and will be included in your first Direct Debit payment. There is no registration fee when joining the children’s or student’s plan.
What is the Dental Accident and Emergency Cover?
Our dental plans include access to a Global Dental Accident and Emergency Assistance Scheme. The Scheme is established to offer support to patients who request assistance or treatment following an accident, a dental emergency, are diagnosed with mouth cancer or who become unemployed following redundancy.
Whilst the scheme aims to provide benefits in most cases, the scheme is a wholly discretionary scheme, not an insurance scheme. It has no obligation to provide benefits and the Scheme Manager will look at each request individually to decide whether or not to provide benefits.

What are the terms and conditions?
Your membership will include various benefits listed above. However, there are some exclusions you should be aware of.
These are:
Dental implant consultations and treatment
Specialist periodontal (gum treatment) consultations and treatment
CFast treatment
Sedation fees
Teeth whitening treatment
Missed or late cancellation of appointment fees
Any dental examinations, dental or hygiene treatments completed prior to the plan commencing
The 10% discount cannot be used in conjunction with any finance agreement
MINIMUM PERIOD
The plan has a minimum membership period of 12 months
We require one month’s written notice to cancel the plan.
Following payment into the plan for 6 months, two dental examinations and two hygiene treatments (or four hygiene treatments on plan 2) will be covered by the plan for that year.
CANCELLATION
The surgery reserves the right to cancel a plan if any monthly direct debit payments fail. Patients may cancel the plan after the initial 6 months has expired by giving one month’s written notice.
